PHS, Inc. is licensed to provide Adult Foster Care in Chisago, Hennepin, and Isanti counties.

PHS, Inc. is also licensed to provide developmental disability services under Minnesota Statutes, chapter 245B.

Our SLS services are provided in two, three or four-person sites.  Individuals enjoy private bedrooms and a comfortable home environment.  With staff support, individuals work to improve their skills in a variety of areas including but not limited to:

• Medication Administration
• Financial Management
• Household Maintenance
• Meal Planning and Preparation
• Community Integration
• Self-Care Skills
• Personal Safety
• Mental Health Stability
• Anger Management
• Relapse Prevention

Individuals participate in a wide variety of community activities such as sporting events, movies, museums, concerts, fairs, vacations, and Special Olympics.
